Does my insurance cover over-the-counter, at-home testing for COVID—19?

Due to the end of the federal COVID-19 public health emergency, over-the-counter (OTC) COVID-19 testing stopped being covered effective May 12, 2023.

However, if you purchased OTC COVID-19 tests out-of-pocket before May 12, 2023, you can still submit a pharmacy claim form for reimbursement for up to 12 months after the date of purchase. To access pharmacy claim forms, select Prescriptions, then Pharmacy Benefits1 to go to your pharmacy’s website and access claim forms. Members with Prime Therapeutics as their pharmacy manager can also access the Prescription Drug Claim Form on Horizon’s website.

While supplies last, you can also order four at-home COVID-19 test kits per household for free from the federal government.
