Complete the Dental Claim Form and mail it, and the required information listed on it, to the address on the form. You can submit out-of-network dental claims by mail only.

View your dental benefits by following these steps:

1. Select Benefits & Coverage, then What’s Covered.
2. Using the Coverage dropdown menu, select the active dental account.
3. Under the Dental tab, go to the dropdown menu titled Service you may need.
4. Select the appropriate dental benefit (such as Dental Crowns or Orthodontics), then Show Coverage Details.
5. Check the chart(s) to see if the benefit is covered, how much your plan will pay, how many visits are covered and if prior authorization is required. You may need to scroll down to view all the information you need.
