  • If you have stand-alone vision coverage through Horizon BCBSNJ, click Vision under Doctors & Care to learn more about your vision benefits, order duplicate member ID cards and look up vision providers who are in-network for your plan.

    Don’t see the Vision link? You may have routine vision benefits covered by your medical plan. This means that you may be covered for routine eye exams (exams to determine the need for glasses and/or contact lenses), vision hardware (such as glasses), and other vision benefits by your medical plan. To see if this applies to you:

    1. Click What’s Covered under Benefits & Coverage.
    2. Select the active medical group number, if applicable.
    3. Under the medical tab, go to the dropdown menu titled Service you may need.
    4. Select the appropriate vision benefit (such as routine eye exam, frames or lenses) and click Show Coverage Details.
    5. Check the chart(s) to see if the benefit is covered, how much your plan will pay, and if prior authorization is required. You may need to scroll down to view all the information you need.
