Use our Doctor & Hospital Finder to find an in-network doctor, dentist, hospital, or other health care professional and follow the steps below:
- Under “What type of care are you looking for?”, choose between Medical, Behavioral Health, Dental or Vision.
- Enter the appropriate health plan and location, if applicable (when you are signed in, the menu defaults to Your Plan).
- Begin typing in search criteria, such as the doctor’s or health care professional’s last name, or name of the hospital, or the specialty. Click from the results that best matches your input.
The results page will show doctors, hospitals or other health care professionals that accept the plan you chose and the information you set, such as city and state. You can find out who is joining and leaving the plan based on your search results. You can also search for a doctor by group practice.
You can view more details about the doctor or health care professional by clicking the View Profile button. Once signed in, you can also write a review, read other reviews and export results to a PDF document to print.

- Using the Coverage dropdown menu, select the active medical account.

- Select the type of service you are looking for, such as Psychotherapy, Psychiatric-Outpatient, or Cognitive Therapy. Then, select Show Coverage Details.

Use the Doctor & Hospital Finder to look up your doctor, hospital or other health care professional.
On the search results page, the tier of the provider you searched for, such as “OMNIA Tier 1” or “Tier 2,” will appear on the right side under each result. Be sure to check the health care professional’s tier status for each office location. Some health care professionals may be part of more than one group practice.
When you choose doctors, other health care professionals or hospitals designated as an OMNIA Tier 1, Inner Circle, or equivalent tier (depending on your plan), you will get the most from your benefits and pay less out of pocket. Check your Plan Details for more information.
